    Position Overview: We are seeking a professional and empathetic Bilingual Care Coordinator to join our team. The ideal candidate will have a strong background in customer service, behavioral health, and administrative tasks, and must be fluent in both English and Spanish. This full-time role (40 hours per week) involves managing client interactions, scheduling, and data entry while maintaining confidentiality and a positive attitude.

    Preferred Qualifications:

    -Associate or Bachelor’s degree in business administration, healthcare, or related field.

    -Experience in behavioral health, substance abuse care, or mental health care.

    -Experience in a customer service-oriented environment (80-90% of job duties).

    -Fluency in English and Spanish.

    Position Responsibilities:

    -Answer patient phone calls courteously and promptly.

    -Accurately schedule and educate patients about services and next steps.

    -Operate within our EHR system (TheraNest) efficiently.

    -Enter and retrieve data using various computer software applications.

    -Conduct patient interviews to collect financial, biographic, and demographic information.

    -Understand and explain patient insurance information and financial requirements.

    -Relay medical information to the clinical team for exceptional patient care.

    -Triage requests and communicate effectively with Directors.

    -Verify and update patient demographic information at each interaction.

    -Communicate scheduling outcomes with referral sources.

    -Handle patient complaints and escalate when appropriate.

    -Report referral numbers daily and monthly.

    -Maintain a collegial and teamwork attitude with all staff and patients.

    -Keep office space orderly and attend mandatory meetings and training sessions.

    -Assist with office coverage as needed.

    Additional Requirements:

    -Ability to work with BIPOC individuals or families and communicate effectively in both English and Spanish.

    -Excellent written and verbal communication skills in both Spanish and English.

    -Ability to work independently and demonstrate a high level of cultural competence.

    -Proficiency in administrative support tasks (scanning, letter writing, mailings, faxing, emailing, Excel).

    -Ability to function effectively in a multitasking, interruption-prone environment.

    -Experience in healthcare facilities preferred.

    Position Overview: Our clinic is on a rapid growth trajectory and we are looking for a human service professional to serve as a Service Facilitator at Anesis Therapy. This position accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress.

    Responsibilities

    - Admits new clients by reviewing records and applications; conducting orientations.

    - Determines clients' requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.

    - Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.

    - Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crisis; providing personal support.

    - Maintains clients' records by reviewing case notes; logging events and progress.

    - Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to the therapeutic team and family; identifying treatment influences.

    - Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.

    - Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.

    - Improves treatment results by studying, evaluating, and redesigning processes; implementing changes; rewriting policies and procedures.

    - Completes documentation and progress notes within a timely manner.

    - Meets budget by monitoring expenses; implementing cost-saving actions.

    - Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant and subsidy applications.

    - Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

    - Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

    - Sign clinical notes within 24-48 hours of seeing a client.

    - Revise clinical notes as required by the clinical supervisor within 24 hours of notice of the need to revise clinical notes.

    Education Requirements:

    Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy.

    Licensed by the State of Wisconsin DSPS, Marriage and Family Therapy Professional Counseling and Social Work, or eligible to sit the Clinical exam.

    Position Summary: Provide home-based family therapy services to families with children 0-17 primarily utilizing MultiDimensional Family Therapy (MDFT) modality in addition to parenting support and skill building, care coordination and resource collaboration. Requires working relationship with Dane County Human Services including child protection and youth justice social workers. This position reports to the program supervisor.

    Additional requirements:

    Applicants must demonstrate an ability to work with BIPOC individuals or families and communicate effectively verbally and in writing. Applicants must have a valid driver’s license, a vehicle in good working condition, and automobile liability insurance that meets minimum agency requirements. Satisfactory completion of a criminal history background check, including no history of DUI violations.

    SKILLS (Minimum):

    -Experience through internship or prior work experience providing counseling services to families, children or individuals within a clinical setting.

    -Serve high-risk families, including those whose children are at substantial risk of placement or who are in placement and cannot be reunified without in-home services. Provide services primarily in client homes. Travel to client homes and other locations.

    -Schedule sessions at times that are convenient for families, and when problems are likely to occur, including evenings, weekends and holidays. Maintain a flexible work week.

    -Participate in weekly team consultation meetings, and individual supervision as needed.

    -Assess safety, structure the environment and use clinical strategies designed to promote safety.

    -Provide a range of clinical, concrete and advocacy services to family members that are consistent with the family’s values, learning styles, lifestyle, circumstances and culture.

    -Develop and maintain a positive, collegial working relationship with family members and system participants.

    -Conduct comprehensive, strength-based and behavior focused assessments through use of MDFT trained module

    -Collaborate with family members and referents in developing intervention goals and service plans.

    -Collaborate and advocate with formal and informal community resources and systems.
